Acromioclavicular (AC) Joints Projections

Patient Preparation

  • Explain the procedure to the patient.
  • Remove jewelry or metallic objects from the shoulder and upper chest area.
  • Position the patient comfortably, standing or seated.
  • Use lead shielding as appropriate.

BASIC PROJECTIONS

1. AP Projection — AC Joints (Bilateral)

Positioning:

  • Patient standing or seated, facing the IR.
  • Both shoulders in the same plane.
  • Arms relaxed at the sides.
  • For stress view (if indicated), patient may hold weights in hands.

Central Ray (CR):

  • Perpendicular to IR.
  • Directed to midline at level of AC joints.

SID:

  • 100–110 cm (40–44 inches).

Collimation:

  • Include both AC joints and lateral ends of clavicles.

Common Pathologies Demonstrated

  • AC joint separation or dislocation.
  • Osteoarthritis or degenerative changes.
  • Trauma to lateral clavicle or acromion.
